Wound and Skin Care Protocol Orders
(For all patients at risk - with Braden Scale score 17 or less).
If MD is a hospitalist, order 21211 is already checked/ordered. Otherwise, only Perineal Derm. and Perineal Cand. and Stage II require MD’s signature. Nurse should initiate other needed interventions ASAP.
